We here at the Office of Defects Investigation at the U.S. Department of Transportation received a complaint that you put in about your 2001 FORD F series. We are currently looking at the spark plug ejection issue and we are at the early stages of the investigation, which we have named DP05-005. How far had you been driving before the incident occurred?
At what speed were you driving when the incident occurred?
Were you accelerating from a stop or decelerating to a stop?
Did you see any warning lights on the dashboard? If so when?
Did you smell anything?
Did you see any smoke or flames?
Did the vehicle lose power?
i. If yes, what was the maximum speed you could drive the vehicle? (Note: If you slowed down the vehicle, do not count this as your maximum speed.)
ii. If yes, did the vehicle stall?
iii. If vehicle stalled, did you try to restart vehicle?
iv. Was the vehicle towed?
Did you still have power steering and power brakes?
How far (minutes/miles) and to where did you drive the vehicle?
Were there any crashes or injuries as a result of the incident?
What did you do once the incident occurred?
Did you open the hood? If so, what damage did you see?
Was the spark plug in the engine cylinder, on the engine, or nowhere to be seen?
Was there any other damage other than engine damage (hood, fuel lines, fluid leaks, etc.)?
Did you see any fluid leaks?
What did the dealer say was wrong with the vehicle?
What repairs did the dealer make?
From which cylinders did the spark plug blow?
What was the cost of the repairs?
Do you have the invoice for the repair? If so, can you fax it to Cheryl Rose, DP05-005 at 202-366-1767?
Can you please send the VIN for your vehicle; we will need this to validate the data you give us for our analysis.
