Looking for a Java programmer...

The Animal Shelter I am volunteer coordinator for is needing a form done up in java to help facilitate ordering of custom engraved dog tags, once we actually get the machine in. It doesn't have to be super fancy one, I've already got the HTML template for the form figured out, but we want to do some fancier things with it before we make it live, for example certain types of tags will be availabel in different colors and sizes, where other tags won't be, and we want it so if they click on one type of tag, it brings up the color options for that tag to select, but if they click a tag which has fewer color options, it only brings up those... same with size... we also want it to be able to display additional lines if the user selects that they purchased the back engraved, but not have those lines displayed if they say no they didn't.
If you are the type that can help with this, and wouldn't mind volunteering some time to a good cause, let me know .
