Help with 04 Expedition EB 4x4 5.4

Two weeks ago I stop and got gas. Before I got home my service engine soon light came on. I stop and check the fuel cap, it was on tight. The next day I went to Advance to read my codes, their code reader was down or something. So I disconnected the battery to see if the SES light would go off. It stayed off for a couple of days and back on. Still havent got the codes checked yet.
Last week I was driving at night and every time I used bright lights my radio would cut off and on. I figured it was the battery or alternator. Went back to Advance to get them both check. The only thing showed up is my battery didnt have enough cold cranking amps. It never fails to crank, but I got a new battery.
Cleaned the cables and instaledl the new battery. In a day the SES light came back on and when using the wipers, heater, ect while the head lights are on the lights get dim. So thinking its the alternator but the battery light on the dash hasnt came on at all and the battery/voltage gauge runs in the middle where it always has.
Going to get the codes check today or tomorrow. Any help, advice, and info would be appreciated. Thanks

Ok went to Advance and the code that came up was B1676 Battery Pack Voltage Out Of Range non-mil and on the rightside of the screen it had ABS $28. They said they wasnt aloud to clear the codes to see if the new battery fixed the problem. They said I could remove the alternator and care it back and they can check it.
I went by another local parts store and they check the alternator while on the car. It tested in the green so its good, even turn on bright light, radio, heater, ect.. and it just drop a bit but still in the green.
Does anyone have any clue in what could be causing that code to come up?
