2004 5.4 Multiple Codes Please Help!

2004 5.4 P0345, P0316, P0308, P0307, P0305, P0304, P0301, P0300, P0174, P0171, P0022, P0021, P0011

I have an older family member with a 2004 F150 XLT 4x4 5.4. This truck have 150k miles on it.

They called me saying it didn't have any power and wouldn't accelerate above 40mph. This truck has had the normal 2004 5.4 issues with noisy cam phasers. I hauled this truck to my house to check it out. It has sit for a couple of weeks, and when I started it, it seemed to run fine. It ran/drove fine until it warmed up. After it warmed up, it started with the lack of power. Once this started, the engine started missing and back firing. I took it straight back to my house, and it wouldn't get above 40. I took it easy limping it home. Once I got home, I pulled the codes that I have listed above. Has anybody encountered this issue, and can you give a good place to start?

It would appear that both cam position sensors are toast. I'd start there and you might find all of the rest go away. If not, repost codes.
You also have a vacuum leak. That's what the 171, 174 codes are. The engine is running lean and can't account for all of the extra air. Most commonly with the 5.4 of your YM, that would be indicative of a bad PCV hose which are a known issue. The hose develops age cracks and leaks at the elbow. A complete unit from Ford should take care of that.
