Passenger Seat Belt in Ratchet Mode

I did a search and did not find my problem so it is with great trepidation that I start a new topic. I have a 2007 F-150 SuperCrew FX4 and the passenger front seat belt is in ratchet mode. It pulls out easily and as soon as it is buckled it ratchets back and will not allow the passenger to lean forward. If you unbuckle and try again it does the same thing, over and over. I thought it was just my wife not fully understanding how a seat belt works, but it happened to my daughter too. And my dad. Where should I start to look for problems? Any help is appreciated.
