VIOLENT shaking

I have a 2004 F150 FX4 Supercrew 5.4L It has 104K miles on it.

When I pulled out on the highway yesterday and hit 45 MPH it started to shake VIOLENTLY. Seemed to be coming from rear of truck somewhere but not for sure. I slowed down and it was still doing it all the way to a stop. Thought a tire was loose and checked all them. I thought it was universal joint going bad. Pulled driveshaft out and checked joints. They are all good and driveshaft seems fine. Balance weights are still on driveshaft too. Funny thing is it did same thing about a year ago and went away after driving about a mile down road. This time its here to stay! In the last 6 months it had seemed to have a vibration at about 60 MPH but not real bad. Has new tires and they are balanced. Checked all front suspension etc, etc. Everything seems to be ok that I can think of to check. HELP

This is an issue I am not familiar with...in what way is the e-brake failing to cause this?

It starts to rub, for whatever reason, causing heat, which causes the vibrations. Same happened on my 04. In my case, all that had to be done was back off on the e-brake adjustment as it was too tight from the factory. When I would put it on, the pedal would only go on a couple of clicks.

Once when my 08' was practically brand new, I had set my e-brake, while launching the boat (or pulling it out) and although I did pull the e-brake release, it didn't fully release... or should I say, the pedal only came up 90% of the way.

So, I got to driving it down the highway, and it started feeling like I had a flat tire, with the whole violent shaking thing. Took me a minute to figure it out, but as soon as I fully disengaged the e-brake, problem solved.
I was worried something might have gotten over heated, warped, or otherwise screwed up, but have not experienced any problems with it since. I've always just been really careful to make sure my e-brake is fully disengaged after using it, ever since then.

Peace,
Fish

pull rotors and check e brake shoes. Saw one that the lining released in drum part which then eventually wedged with other shoe and caused shuttering.
