Intermediate shaft replacement...Help!!

I've been a longtime lurker here on the forums, and have learned a lot.
But after reading all the post regarding the Intermediate shaft for the 2004's and the TSB's for it, I'm stumped and have to ask for help.

I'm having all the issues such as very hard steering and the clicking sound from the wheel. I went to my local Napa and picked up the upper Intermediate shaft as the TSB says to replace.

Do I need to removed the steering wheel? And where exactly is it located?
I bought a manual for the truck but it doesn't go over the Intermediate shaft very well.

If anyone has had to replace this could you help me out?
