Does anyone have the link to the Ford site where you can download/print out the form that entitles you to a $50.00 Cabela's gift certificate when you test drive an '04 F-150? There was a post on here not long ago with the direct link but I can not located it anymore. Lost my bookmark. Tried every search variation but am not seeing it. The post started out something to the effect of "Not sure if you guys have seen this yet..."

I sent mine in 2 months ago and still haven't received the gift certificate. You might not want to waste your time.

Recieved my $50 gift certificate from Cabela's in todays mail. I took my test drive about 2 months after I got my 04 lariat.

 

So does anyone actually have a web page link for this GC? Or a printed form that can be faxed?
