There's no adjusting and no, they don't have to be lifted at the same time. Once you pull all the cab and bed bolts out and it is just sitting on the frame, you will want to take the bed and slide it back about 6" or so. That way when you lift the cab, you won't hit the bed. Lift the cab first, then the bed. They should all line back up once you have the 3" nylon Spindles in place and drop in the new body bolts.

I used a floor jack and some pieces of wood to lift the cab and bed.

you can lift them them at different times, just have to scoot the bed back a few inches. All it takes is time, a friend and a breaker bar on some of the body bolts. I did the instal over a couple of free days, i would set a weekend aside to get it done and do it yourself. I couldnt imagine paying to have it done either.

A 2 or 3 ton floor jack would be fine and some wood pieces, and the steering extension is bolted on after the lift. You disconnect it before oyu lift then install the extension after the body is bolted back on.

You have to take the lower section of the back seat and fold back the carpet in order to get to them. There are 6 in the cab. 2 under the back seat, 2 in the floor in front, and 2 in front of the engine.

how do you get to the ones in the middle? Also when you move the bed back, do you just drop it right down and it will rest safely or do you leave it up?

The carpet just folds back, but you will have to take off the kick panels and what not. The instructions will explain EVERYTHING and then some.. trust me its easy. You arent sliding the bed back that far so no.. just loosen the bolts and pull them. It will sit right there.

Also, whether or not you have to remove the rear seats depends on your cab style. I didn't have to remove the seats in my Screw, but I had to remve the second and third row seats in a friends Expedition.

In screw cabs you dont even have to remove the carpet in the back, you can just pull it up and stick the bold in without even looking really. And as far as time wise goes..... if you have a general knowlage of trucks and what not, you can do it in about 8-10 hours. It can be done in one day if you start early enough.
