What year is the truck?
What codes by number, are you getting?
The Ox sensors, late ignition timing blocked cats could be contributing to the poor fuel milage and performance.
Opening the PCV hose to air will make the idle go high; it's normal action.
You have to check each areas out to be sure and not guess by replaceing parts without some indication of what is faulty.
The EGR valve has a control on it's operation as well as the EGR it'self can be faulty.
Takes some detective work if your going to try the repairs.

There is no code 43 for your truck. You might want to check the codes again. Otherwise, double check the vacuum lines associated with your EGR valve for cracks or leaks.

At 5 mpg in unloaded conditions, the exhaust would have to dam near be smoking.
I get over 9 pulling combined weight of nearly 12,000 lbs towing.
