Cam phaser codes with new phasers

We have a 2004 F150 not the heritage that HAD a 5.4l v8 3v motor. Motor blew up and we put in a good 2006 explorer 4.6l 3v motor. Motor ran perfect 5000 miles then started having phaser problems. Motor has 130xxx miles on it.

changed both cam phasers, cam phaser oil control solenoids, solenoid housing gaskets, oil pump, timing chains cassettes / tensioners / chains / gears etc. put new ford spark plugs in it when we put motor in it. Oil pressure gauge always shows fantastic pressure. We used mobil 1 full synthetic 5w-20 oil and a motorcraft filter.

still getting dtcs for cam phasers/ control solenoids: p0021 (bank 2), p0022 (bank 2) and sometimes and p0175 bank 2 rich. We get the p0022 the most with an occasional p0021.
we have replaced the cam position sensor on the drivers side.

anything else cause these codes? Its driving us crazy... motor had zero sludge in it when we did the timing parts and valve cover gaskets.

Anyone have any suggestions? Pretty sure the computer is shutting down the vct on the left side causing this but not sure why. When we tore down the timing on this the drivers side tensioner had a blown out gasket. We changed everything timing related and the oil pump.

Could this be a bad crankshaft position sensor? We have replaced or swapped both cam sensors already.

we are running a motorcraft oil filter and 5w-20 oil as many have said to do because of these problems.
