Fueling questions

So over the time I have had this truck I have ran into different fueling issues. First being a few months back I got the low fuel rail psi code (can't remember the number) but changed the filter and checked the fpdm and everything look good. My rail psi will randomly dip low and bounce back on the next drive usually. However it's been staying low the last couple days but now I'm getting a stuck rich code. P2196/2198.
Rich and low psi sounds like injectors no?
04 5.4

Or am I looking at more of a tune issue? Never had an issue stock but can't flash it back to stock (phaser lock outs) but would also make sense that I have allowed fuel psi and running rich if I'm using too much fuel.

It sounds like a failing fuel pump.

its crazy, it will be fine for weeks and then tank the psi to the low teens, just got back from a 600 mile trip and it was fine, running around town and it starts. however when i got the last round of codes and the o2 stuck rich it was 115 out. hot outside,1/4 of fuel and an aging fuel pump starts to show itself?

Only thing you might run into is no fuel gauge.

Found a deal on a airtex assembly and just popped for it. This truck is not one to see a lot of use but when it does it's going 300 miles into the middle of nowhere. Not the place to skimp on a couple bucks. Will have to drop the tank next week.

Thanks for the pointer on the fuel pump, got the new one in today and the old ones sock was torn and my duty cycle went from always being .48 to .12 and fluctuating like it's supposed to under load.
