One way to look at all the problems you've had so far is that eventually everything will have been fixed and there won't be anything left to break.

Running lean from the combination of a cold air intake, aftermarket exhaust, and stock tune?

From my limited knowledge of engines, my answer would be yes... but anyways, here is my setup:

I've never had it tuned... I have the E85/Gas 5.4L V8 (P.S. The truck I liked just happen to have that, and just happened to be the one I bought). I have an AiRaid Cold Air Intake and Flowmaster 50 series SIDO. That's it as far as the engine and mods to the engine go...

Welp, I got my truck back this afternoon. Needless to say, I am very thrilled to be driving my F150 after driving my brother's Tahoe for more than a week.... Man I love the way it sounds, lol... But they said that they fixed everything, and that they couldn't find anything wrong with the transmission. I drove around for about an hour after I picked it up and it was still downshifting hard...
