REEL Error on facory display

REEL Error on factory display
Anytime I insert a cassette into the factory (AM/FM CASS/CD CHNGR) I get a "reel error" on display.
This is on a 99 Lightning and just started about a week ago.
I use a cassette adapter to a mini plug from my MD player. It also happens with any cassette.
Can't find any information in manual- any ideas?
