I've seen them out in the mountains of Western MD pulling big trailers (gooseneck or 5th wheel? Not sure on the terminology) for both camping and horse hauling.

A Gooseneck hooks to a heavy duty 2-5/16" ball mounted in the bed of a truck. You can convert fifth wheel to gooseneck with an adapter plate set up,-if need be. What sucks, you must position your ball directly under the hitch, which is not as easy when the ball is centered over the axle. Specially when you can't see the damn thing lol. Takes a little skill.

The 5th Wheel hangs down from underneath and has a big steel plate with a 2" pin that aligns it's self (within reason) with a big plate on top of your 5th wheel hitch set up in your truck. A much nicer set up.

BTW, -I hope that helped lol. Here, short version ~

Goose-neck = Ball hitch, usually 2 5/16"

5th Wheel = A pin drop, usually 2"

Both in the bed. Top of rear axle.

I'm not sure I follow you on that one PatMan? Maybe it is just you...

What I do find funny is seeing a big rig towing a lowboy (I do know that trailer terminology, Jbrew! lol) with a small tonka toy bulldozer strapped down on the lowboy. I see that every now and then on our highways.

At 0:58 "welp, it's gettin' a bath!"

Painful to see that. Even if it is a Dodge.
