Truck going in for service

What should I try to have done?

I have the rear end clunk on take off, a nasty clunk/rough downshift when taking off from a slow speed (10mph or so), poor poor throttle response and acceleration, and a 5.4 that sounds like a diesel.

Hopefully a PCM flash and slip yoke re-grease and be all set?

The reflash and slip yoke grease should solve your issues. I've got the clunk on take off. Wonder if I can grease that thing myself?

Dealer called, they didnt fix anything, had the truck for two days. Said nothing is wrong, everything is normal.

It clunks on take off, it clunks around turns, it clunks when downshifting, but they said it doesnt do it. The headliner rattles, they said its fine. The throttle response is normal and the engine sounds that way because of a cheap oil filter. WTF?

I took my truck in earlier this year for throttle response and the diesel sound also (and not cold A/C and rattling rear window). The throttle response was handled with a computer reflash and the diesel sound was due to noisey cam phasers (mine was the right side).. I think the reflash took care of my rare clunk sound (perhaps it tightens up the tranny shifts so it reduces the clunking in back?)

I'd take it to a different dealer and see what they say. If they end up fixing all your problems I'd complain to the original dealer and if you had to pay for a rental at the first dealer I would ask for a refund and threaten to complain to the regional manager if they don't give you one.
