This applies to any vehicle; was the flywheel bolts torqued to spec?
If automatic trans, was the converter bolts to spec?
Does the starter need a shim?
Is the starter engaging the flywheel properly? Remove the starter and look for signs on the flywheel of interference.
The starter gear may be hitting the flywheel more than just gear/teeth engagement.
One or more of the above has to be the cause.
Good luck.

Why are you asking about a 1991 Chevy van in a 1997-2003 Ford F-150 forum?

The only appropriate forum for that here might be General Discussion.
