serpentine belt misalignment 1999 F-150 5.4

What's happening?

Has anyone noticed that the belt that spans from the tensioner to the crankshaft pulley is running on an angle, causing the belt to favor the back of the crankshaft pulley?
I don't know what could be causing this...nothing was changed. Replaced belt and tensioner..no luck.
Any help, please!

Did you replace the tensioner arm, the complete assembly or just the pulley? The pulley bearings go bad and can cause mis-allignment of the belt. If the assembly was replaced there is a peg that needs to be aligned when it is bolted on that can cause mis-alignment as well.
