may need clutch but for changing the tranny fluid there is a drain plug on the driver side of the tranny and a filler plug as well just drain the bottom plug then put new fluid in the top plug. make sure the fluid overfills a little bit.

if it shifts into all other gears especially 1st and rev the clutch is fine and you have an internal trans problem or posibly a problem with the shifter

also when changing fluid i find it much easier to remove the shifter and fill the trans from the top in the cab also if you do this you could try shifting the trans into 5th with a large screw driver or metal rod to make sure your shifter is not binding up if it still shiftes harder than the other gears its internal and to further investigate you would need to remove the foor plate thats under the carpet and remove the top of the trans and inspec the shift forks and
5th syncro/sliders
