2002 Hesitation (slip) on take off

My 02 F150 4x4 has been having a problem with hesitating when you take off. I let off the brake and the truck will not idle forward....then I give it gas and I get a thump before it starts going forward. It doesn't happen all the time....when it warms up it will start doing it. I took it to the dealer who changed the grease on both ends of the driveshaft, but it didn't help. I had the trans fluid changed at the dealer but still no help.
Any suggestions.....it has 167k on it.

How is your 1-2 shift? My 97 was doing the same thing you described and the 1-2 shift would be erratic normally when the transmission was cold it shifted fine but once it warmed during light throttle acceleration I would have to either let off the throttle or rapidly press the throttle to get the 1-2 shift. Well last week I found out that that is an indication that the direct clutch is dying/dead. Friday I was left a stop sign engine rpm got to 2500 and the truck felt like it fell out of gear then all I had was 1st gear. Today transmission shop is rebuilding my transmission.

Has anybody heard anything new about this? I have an '01 Supercrew 5.4 that has started doing the same thing, only when cold, but it will stop once the tranny warms up. I had the tranny flushed and it got better, but it's still there.
