Tranny/Engine

Help please. I have an 05 4x4 F150 SCREW and she has 45000 miles roughly. I recently started having a problem on start-up. It is a knocking/clunking sound. At first I thought it was the idler pully or the tensioner. Its not. Then I changed the starter because it sounded as if it was engaged too long. It was. But the sound has still not gone away. I was under the truck and I had my wife crank it for me and it sounds as if it is coming from the tranny. The really weird thing that throws me off the trail of the problem is that it only does it if it is a 1st start for the day of it the truck sat for awhile. Can someone help me please. All inputs would be greatly appreciated.
