Cluncking noise and jerking during up and downshifts

Hey guys, I hope someone can help me with this. i know this has to have happened to someone before.

The rig:
1997 F150 flareside
5.4 Auto 4X4 Xcab

Problem: This has started to happen since about a week ago. When accelerating from 1 -2 -3 there is a sudden jerk at each shift if i'm accelerating at part or half throttle, but the truck accelerates fine and smooth when accelerating full throttle. It also gives same sudden jerk when I letoff the accelerator while in motion and the tranny has to downshift to lower gear. I don't notice any dummy lights coming on either.

Can anyone give me a hand? Any inputs will be greatfull.
