Maybe nothing. Cleaning the MAF may fix both!

+100, pcv elbow is most common, running lean raises cyl temps and egts and can cause detonation, not good for your engine.

there is a hose comming out of the passenger side valve cover, follow that hose to the back of the intake and you will find the elbow.

see it coming out of the valve cover is the elbow hook into the upper intake behind the throttle body? i dont hear a vac leak, if the maf has been cleaned and the elbow isnt leaking is there any other thing could be fault for the leaning condition?

this is where a scan tool is very handy, to know why you are running lean it also helps to know WHEN it is running lean(freeze frame data is key here). also just because you have cleaned the maf does not mean it is no longer a suspect, cleaning doesnt always work. have you checked fuel pressure? Im sorry to say but you may have done all you can without the proper equipment and a trip to the dealer may be in your future.
