I know exactly what you are talking about. I am dealing with the same thing. At 40-45 and 1100-1500 RPM I get a nice little shudder/jerk feeling, this only happens on slight inclines or very light acceleration and it seems to be right before the transmission should shift. Are you in OD when this happens like I am? I have also felt the same problem when on the interstate traveling about 70 mph at 2000 RPM and again on an incline or steady throttle accel.
I had a similar problem once before with a shudder/jerk/miss kinda feel before with lower RPMS and it steadily got worse and threw a code. That time it was plug wires arcing. How long has it been since you checked/changed the wires and maybe the plugs? I only had about 24K on that set (aftermarket wires) before they were junk. Put on Accel wires and coil packs and she ran great up until about 2 months ago.
This time it feels more like a tranny problem to me but I had one Ford tech to say it felt like a dead spot (miss) in the engine just at that one spot. I have had my tranny flushed and a shudder fix added to no avail.
Let me know if you find anything out. I myself might try to change the plugs and recheck the new wires. From there I might have AAMCO do their free diagnostic on it. I also plan to have the professional fuel injection service done at a dealer.
