hey 4.2L Guy i got a 98 auto reg cab long box and i get 20 mpg highway and 16-17 in the city the first thing i would be checking is do you have factory plugs in it aftermarket plugs are junk in ford they dont like em and are your wires somewhat new. Check your coil if your not sure start your motor and put on a welding glove and pull each wire off the coil and see if you see a good arc from the coil pack to the wire, the exhuast hurts you with out a dout in the city i live in toronto canada so its smog and start and go too. you should get atleast 17 to 18 highway with a standard you should get better then me thats for sure. the only other thing i would suggest is to check your o2 sensores if the through a code every once and a while the will take you down 1mpg aswell and clean your throttle body aswell. oh and the air filter upgrade. i had one it didnt do anything for power and just hurt gas millage. More air going in equals more fuel going in.

New Factory plugs
The more expensive wires at your local auto parts store or factory
Check O2 sensors
Check coil might be going aswell
Clean Throttle body
K&N air fitler NO GOOD (i run a fram platinum)
Rear Brakes dragging (Facotry PBrake cables arent that expensive but if you use it everyday it shouldnt seize)
Tire Pressure the harder you run em the better MPGs I run my fronts are 40Psi and my rears at 38psi.
