Kaptain did a good job on your list.
I also just had this done. There was a couple things that differed but most of it was the same.
I spent $5000 + shipping on my trany!($1000 was core deposit. Had nothing to send them) I tried to get one from my local junk yard and they were all 1000 bucks so I just said F it..lol
In the end it cost me between 8 and $8500.
